 An interesting study is called, Ultrastructural comparison of an adenomatoid tumor, lymphangioma, hemangioma, and Mesothelioma by Joseph B. Marcus MD,, Joseph A. Lynn MD, &#8211; Cancer Volume 25, Issue 1, pages 171175, January 1970. Here is an exerpt: Abstract &#8211; An electron microscopic study was made comparing an adenomatoid tumor with a hemangioma, lymphangioma, and 2 mesotheliomas in an attempt to elucidate the cell of origin of the adenomatoid tumor. Using ultrastructural criteria, one could say that neither the hemangioma or lymphangioma were structurally related to the adenomatoid tumor. It was not possible to morphologically distinguish between the cells of the mesothelioma and that of the adenomatoid tumor.</p>
<p>Another interesting study is called, Phase II trial of high-dose cisplatin in patients with malignant mesothelioma. By Mintzer DM, Kelsen D, Frimmer D, Heelan R, Gralla R. &#8211; Cancer Treat Rep. 1985 Jun;69(6):711-2. Here is an excerpt: Abstract &#8211; Twenty-five patients with advanced malignant mesothelioma were treated in a phase II trial with high-dose cisplatin (DDP). All patients had measurable or evaluable disease. Seven patients had received prior chemotherapy. DDP (120 mg/m2) with mannitol and prehydration was given every 4 weeks for two doses and then every 6 weeks. Of 24 patients evaluable for response, three had partial responses (3, 3+, and 9 months; response rate, 13%; 95% confidence limits, 4%-31%) and one had minor response. All responses occurred in previously untreated patients. High-dose DDP has modest activity in malignant mesothelioma.</p>

<p>To determine whether mesothelin can assist in discriminating epithelioid mesotheliomas from lung adenocarcinomas or from other carcinomas metastatic to the serosal membranes, 55 mesotheliomas (44 epithelioid, 3 biphasic, and 8 sarcomatoid), 48 carcinomas of the lung (31 adenocarcinomas, 17 squamous carcinomas), and 86 nonpulmonary adenocarcinomas (14 ovary, 5 peritoneum, 9 endometrium, 11 pancreas, 4 stomach, 16 colon, 12 breast, 9 kidney, 4 thyroid, and 2 prostate) were investigated for mesothelin expression using the recently available 5B2 anti-mesothelin monoclonal antibody.</p>
<p>Another interesting study is called, Efficacy of cisplatin-based intraperitoneal chemotherapy as treatment of malignant peritoneal Mesothelioma by Maurie Markman and David Kelsen &#8211; Journal of Cancer Research and Clinical Oncology &#8211; Volume 118, Number 7, 547-550. Here is an excerpt: Abstract &#8211; In an effort to examine the potential clinical utility of intraperitoneal (i.p.) therapy in the management of patients with malignant peritoneal mesothelioma, 19 individuals with this disease were treated with a cisplatinbased i.p. treatment regimen. All but 1 patient also received i.p. mitomycin. The treatment was generally well tolerated, although a maximum of only four or five courses of cisplatin (100 mg/m2 every 28 days) and mitomycin (510 mg/treatment given 7 days after each i.p. cisplatin administration) could be administered, the treatment principally being stopped because of disease progression or catheter failure. Of 15 patients with malignant ascites, 7 (47%) experienced control of fluid reaccumulation ranging from 2 months to 73+ months (median 8 months). While the median survival for the 19 patients was only 9 months, 4 (21%) patients survived for more than 3 years from the initiation of therapy, and 2 patients are currently alive and clinically disease-free more than 5 years from the start of the i.p. treatment program. We conclude that a subset of patients with peritoneal mesothelioma, principally those with small-volume residual disease following surgical tumor debulking, can benefit from a cisplatin-based i.p. treatment strategy with control of ascites and prolonged disease-free survival.</p>

We have developed and validated Modified RECIST criteria adapted to the growth pattern of malignant pleural mesothelioma. Patients and methods: We evaluated 73 patients from two clinical trials of cisplatin/gemcitabine chemotherapy in malignant pleural mesothelioma. Tumour thickness perpendicular to the chest wall or mediastinum was measured in two positions at three separate levels on thoracic CT scans. The sum of the six measurements defined a pleural unidimensional measure. Bidimensionally measureable lesions were measured unidimensionally as for RECIST. All measurements were added to obtain the total tumour measurement. A reduction of at least 30% on two occasions 4 weeks apart defined a partial response; an increase of 20% over the nadir measurement, progressive disease. <p>Pleural mesothelioma is a type of cancer that develops in the lung&#8217;s outer linings &#8211; the pleura.</p>
<p>Your mesothelium is a large covering of tissue that protects a number of your body&#8217;s organs in both your abdominal cavity and your chest cavity. Your pleura is the two-layered membrane surrounding your lungs.</p>
<p>The inner layer of the pleura surrounds your lungs. The outer layer connects to the wall of your chest. The small space between the two is the pleural area.</p>
<p>Approximately 70% of all cases of malignant mesothelioma are pleural mesothelioma.</p>
<p><span id="more-128"></span></p>
<p>What Causes Pleural Mesothelioma?</p>
<p>Usually people absorb asbestos through inhalation. Once inhaled, there is no physiological process through which the human body can get rid of asbestos fibers.</p>
<p>The sharply edged asbestos fibers usually lodge themselves in the lungs and, over time, work their way to and embed themselves in the pleura. </p>
<p>When this happens the fibers will cause the mesothelium cells to grow uncontrollably.</p>
<p>The walls of the pleural will thicken. This will cause fluids to build up. Eventually tumors will develop.</p>
<p>Because this process often takes decades, mesothelioma usually isn&#8217;t diagnosed until many years after someone was exposed to asbestos. It&#8217;s a slow process that can take twenty to forty years. But, eventually, it may cause symptomatic developments.</p>
<p>Once the mesothelium starts to swell, the space between the chest wall and the outer lung starts to narrow.</p>
<p>Until that time the body provides a thin layer of fluid between the surfaces. The fluid permits them to slide against each other.</p>
<p>However, the fluid dissipates because of the swelling. </p>
<p>When that happens the mesothelioma victim feels acute discomfort with every breath because the surfaces of the now-dry walls rub against each other.</p>
<p>The opposite effect can be caused as well, and that&#8217;s when excess fluid takes up the pleural space. This is called pleural effusion. The excess fluid makes it difficult to take in a full breath of air. If this happens excess fluid can initially be drained non-surgically. This will reduce the patient&#8217;s discomfort.</p>
<p>Eventually, though, this condition will develop in eighty- to ninety-percent of pleural mesothelioma victims. Of those, about 60% feel the discomfort &#8220;only&#8221; on the right side of their chest cavity.</p>
<p>Pleural mesothelioma can be particularly difficult to treat using standard radiologic therapy and standard chemotherapy because it is a diffuse type of cancer. Because it consists of a lot of small tumors rather than one large tumor there isn&#8217;t a single large mass that can be targeted.</p>

Using immunohistochemical analysis with D2-40, a recently available monoclonal antibody that has been used as a lymphatic endothelial marker, we examined 53 cases of mesothelioma, 28 cases of reactive pleura, 30 cases of pulmonary adenocarcinoma, 35 cases of renal cell carcinoma, 26 cases of ovarian serous carcinoma, 16 cases of invasive breast carcinoma, 11 cases of prostatic adenocarcinoma, and seven cases of urothelial carcinoma. </p>
<p>In addition, immunohistochemistry using calretinin, cytokeratin 5/6, and WT1 was performed on all cases of mesothelioma, pulmonary adenocarcinoma, ovarian serous carcinoma, and renal cell carcinoma. Predominantly, membranous D2-40 immunoreactivity was present in 51 of 53 (96%) mesotheliomas, 27 of 28 (96%) cases of reactive pleura, and 17 of 26 (65%) ovarian serous carcinomas; membranous staining was not seen in any other tumors examined. Compared to other immunohistochemical markers of mesothelioma, D2-40 was as sensitive as calretinin and more sensitive than cytokeratin 5/6 and WT1. We conclude that D2-40 immunoreactivity is sensitive for cells of mesothelial origin, and may be useful in the differential diagnosis of epithelioid malignant mesothelioma vs adenocarcinoma.&#8221;<span id="more-126"></span></p>

Since the introduction of epitope retrieval methods, the sensitivity of many antibodies has been enhanced. Thus, a reevaluation of the mesothelioma/adenocarcinoma diagnostic panel becomes necessary. We studied 268 paraffin-embedded formalin-fixed tumor samples that included 57 epithelial mesotheliomas and 211 adenocarcinomas of various origins, comparing an extensive antibody panel with and without heat-induced epitope retrieval (HIER). Marked increase in the sensitivity of several antibodies, with no loss of specificity, was found when HIER was used. After statistical analysis, the antibodies to the epithelial glycoproteins carcinoembryonic antigen, BerEp4, and Bg8 emerged as the best discriminators between adenocarcinoma and epithelial mesothelioma within the entire panel. The mesothelium-associated antibodies, HBME-1, calretinin, and thrombomodulin were less sensitive and less specific than the former, although they were found to be useful on certain cases. Antibodies to cytokeratins and vimentin, although of minor diagnostic value in this context, may be helpful to evaluate the quality of antigen preservation. This study confirms the value of immunohistochemistry to accurately distinguish mesothelioma from adenocarcinoma when an antibody panel approach is used. The addition of heat-induced epitope retrieval methods increases the effectiveness of the procedure and is recommended for most of the antibody panel members.&#8221;</p>

Recent data noted in vivo antineoplastic effects of a combination of mitomycin and cisplatin (DDP) greater than either agent alone in human tumor xenograft mesothelioma. While DDP singly has some efficacy, mitomycin has not been systematically evaluated in this disease. A phase II trial was therefore conducted to determine the activity of mitomycin in malignant mesothelioma. Pulmonary toxicity was substantial. Two responding patients developed dyspnea and hypoxemia occurring at cumulative doses of 40 and 50 mg/m super(2), respectively. One patient had a transbronchial biopsy consistent with drug-induced pulmonary fibrosis and never achieved resolution of hypoxemia with corticosteroid therapy prior to death from progressive mesothelioma. The second patient responded to corticosteroid therapy with resolution of dyspnea and hypoxemia but relapsed after discontinuing mitomycin.&#8221;</p>

The anticytokeratin antibody reacted strongly with all cases of reactive mesothelium, mesothelioma, and lung carcinoma. Antibodies to human milk fat globule membrane and the Ca antigen stained mesothelioma and carcinoma and 43% of cases of reactive mesothelium. Staining for carcinoembryonic antigen was not detected in reactive mesothelium or mesothelioma, but was present in most of the lung carcinomas. CEA seemed to be the single most useful marker in distinguishing carcinoma from mesothelioma in that a positive reaction for CEA would indicate carcinoma rather than mesothelioma.&#8221;</p>

Immunohistochemical study of podoplanin was carried out in five malignant mesotheliomas and 118 other tumors including 93 adenocarcinomas, four squamous cell carcinomas, six gastrointestinal stromal tumors and five endocrine tumors. Immunoreactivity for podoplanin was demonstrated on the cell membrane of tumor cells for all mesotheliomas. All other tumors were negative for podoplanin. Among the many antibodies used for differential diagnosis of malignant mesothelioma, podoplanin has the potential to be an excellent tumor marker in both specificity and sensitivity. The utility of podoplanin as a marker for mesothelioma will be confirmed by further studies.&#8221;</p>
